New Patient Registration
You may register as a patient if you live within the practice boundary and if the practice list is open by completing a new patient registration form and a health questionnaire. These are available from reception or via the above “Registering Online” tab.
If you have a chronic health problem such as diabetes, breathing problems, heart disease, epilepsy, and blood pressure problems or stroke, please make an appointment to see Janette Sammons our healthcare assistant. If you are in good health and are not on any regular medications you will not need to see anyone before making appointments in the normal way.
At your first appointment the doctor/nurse will take some basic measurements such as height, weight, and blood pressure.
All new patients needing repeat medications will need to see the GP/nurse clinician.
