Patient Information

PATIENT NOTICE!!!
WEIGHT LOSS INJECTIONS
The NHS has been told to prepare services for patients wanting to lose weight using weight loss injections (Mounjaro). An original target date was set for the 23rd of June for everything to be ready but plans have not been completed yet and a service is not ready for now or for then.
The NHS will be starting with patients with the highest risk before making it more available to others but this will be a long process over years due to the high numbers.
The current guidance is that they will start with patients who have a BMI of 40 or above, PLUS four or more of the following health conditions
- high blood pressure (on treatment),
- high cholesterol levels requiring treatment (or high levels without treatment),
- obstructive sleep apnoea needing treatment such as CPAP,
- cardiovascular disease (such as ischaemic heart disease, strokes, heart failure, peripheral vascular disease)
- type 2 diabetes.
We are not able to start this for anyone who does not reach this criteria set by the NHS and that is only when a service is set up and offered for GPs to run.
Please don't arrange an appointment to ask for Mounjaro for weight loss at the moment, as we are not able to start it yet.
Please follow the link below for further guidance
Thank You

Welcome to Five Oaks Family Practice

With patients' needs at the heart of everything we do, our website has been designed to make it easy for your to gain instant access to the information you need. As well as specific practice details such as opening hours and how to register, you'll find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations.

We hope you enjoy having a look around the site and familiarising yourself with some of the online features, such as ordering a repeat prescription, booking and cancelling of an appointment, and registering with the practice.

Whatever your thoughts, be sure to let us know via our feedback function. Comments and suggestions are always a great way to help us continue to enhance the way we look after you.

Training practice

We are a training practice and provide placements for GP registrars and student nurses.  You may be offered an appointment with one of our Registrars and from time to time they may ask permission to video the consultation.

Students may be present during an appointment you have with your GP or Nurse. If you have any objections to any of the above, please let the Reception staff, GP or Nurse know.

Appointments

It is important to let the practice know if you cannot attend an appointment or no longer need a telephone call
The number of DNAs (did not attend) is increasing
In July 2025, there were 76 DNA's with the nursing team and 151 DNA's with the GP’s, 7 of those appointments were booked on the day as an emergency.
Without informing us, we cannot offer this appointment to a patient who needs it which increases waiting times for all and wastes valuable NHS time.
If a patient fails to attend an appointment without notice, a text message orletter will be sent.
If a patient fails to attend a second appointment without notice within a 12 month period, a further letter will be sent
If a patient fails to attend a third appointment within the same 12 month period, a decision will be made by the partners and management as to whether the patient is removed from the practice list
All letters are saved in the patient's health record and are valid for a 12 month period

Prescription Requests

From 1st July 2023 the practice will no longer be accepting prescription requests over the phone, unless you are over 80 or housebound. These reqeusts will be taken between 11.00 and 12.00 noon. For patients who do not meet this criteria please bring your request to the surgery or alternatively, you can email the practice to gmicb-mh.fiveoakspxorder@nhs.net

We encourge our patients to register on the NHS app to order their medication.

The NHS App provides a convenient way to request repeat prescriptions. Patients can order their medications through the app and have them sent to a pharmacy of their choice.

Here's a breakdown of how it works:
1. Access the NHS App: Download and log in to the NHS App on your mobile device or access the NHS website.

2. Locate the Repeat Prescription Service: Find the "Repeat Prescriptions" section within the app, usually on the homepage.

3. Select Your Medications: Choose the medications you need from the list of repeat prescriptions.

4. Choose a Pharmacy: You can nominate or change your preferred pharmacy within the app.

5. Request the Prescription: Confirm your choices and submit your request.

6. Track Your Request: You can view the status of your prescription request in the app, including whether it's been approved.

7. Collect Your Medication: Once the prescription is approved, you can collect your medication from your nominated pharmacy.

Key benefits:
  • Convenience: Order prescriptions anytime, anywhere, without needing to contact your GP directly.

  • Choice: Select your preferred pharmacy and have your medication delivered there.

  • Control: Track the progress of your prescription requests within the app.

Friends & Family

The Friends and Family Test (FFT) is a survey used by the NHS to gather patient feedback on their experiences with healthcare services, including primary care practices like Five Oaks Family Practice. Patients are asked how likely they are to recommend the service to friends and family, and can also provide comments to explain their score and suggest areas for improvement.

Here's a more detailed explanation:
  • What is the FFT?
It's a simple questionnaire designed to capture patient views on their care.

  • How is it used?
The FFT asks patients about their overall experience and their likelihood of recommending the service.

  • Why is it important?
The feedback helps healthcare providers understand what is working well and where improvements can be made, ensuring better care for all.

  • At Five Oaks Family Practice:
You can expect to be asked questions about your experience at Five Oaks, and you'll have the opportunity to provide detailed feedback.

  • Anonymity:
The FFT is designed to be anonymous, so you can share your honest opinions without worry.

  • How to participate:
You may  receivce a text message after you have visited the practice or given a survey form to complete.
