Welcome to Five Oaks Family Practice

With patients' needs at the heart of everything we do, our website has been designed to make it easy for your to gain instant access to the information you need. As well as specific practice details such as opening hours and how to register, you'll find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations.

Training practice

We are a training practice and provide placements for GP registrars and student nurses.  You may be offered an appointment with one of our Registrars and from time to time they may ask permission to video the consultation. 

Students may be present during an appointment you have with your GP or Nurse. If you have any objections to any of the above, please let the Reception staff, GP or Nurse know.

Appointments

It is important to let the practice know if you cannot attend an appointment or no longer need a telephone call
The number of DNAs (did not attend) is increasing
In July 2024, there were 171 DNAs with the nursing team and 180 DNAs with the GP’s.
Without informing us, we cannot offer this appointment to a patient who needs it which increases waiting times for all and wastes valuable NHS time.
If a patient fails to attend an appointment without notice, a text message orletter will be sent.
If a patient fails to attend a second appointment without notice within a 12 month period, a further letter will be sent
If a patient fails to attend a third appointment within the same 12 month period, a decision will be made by the partners and management as to whether the patient is removed from the practice list
All letters are saved in the patient's health record and are valid for a 12 month period

Prescription Requests

From 1st July 2023 the practice will no longer be accetping prescription requests over the phone, unless you are over 80 or housebound. These reqeusts will be taken between 11.00 and 12.00 noon. For patients who do not meet this criteria please register on the NHS app, bring your request to the surgery or alternatively, you can email the practice to gmicb-mh.fiveoakspxorder@nhs.net

Covid-19 Vaccinations

The practice is now offering Winter Booster Covid-19 Vaccinations. Patients will recieve a text message inviting them to the clinics.

Vaccination programmes

Seasonal Vaccinations 
RSV (Respiratory Syncytial Virus) Vaccine
This is a new vaccine for patients once they turn 75 years of age, they remain eligible until they turn 80 (this can be given all year round)
From September there is a catch up campaign for everyone aged between 75 & 79 years (on 1st Sept 24) The RSV vaccine protects against lung & respiratory tract infections. Pregnant ladies are also eligible after 28 weeks, please see your midwife. This vaccine cannot be given at the same time as any other vaccine.
Flu Vaccine
In September we will be starting to invite all 2 & 3 year old's & pregnant ladies (20 weeks plus) for their flu vaccines.
NHS England have advised we cannot vaccinate other eligible groups until October.
All our flu vaccines are ordered for eligible patients, please support your Practice by having your flu vaccine here.
Anyone eligible for pneumonia or shingles vaccine will be offered also.
Invitations have now been sent to all eligable patients. Please make an appointment at the surgery.
